Yearbook Distribution

Name Change Form

Do you use a different name at school than what Seattle Public Schools (SPS) has on record? You may submit a yearbook name change form at the start of the school year to request that we use a different name for you in the yearbook than the name that is registered with SPS. Check back here in September for more info about how to submit your name change.

Questions? Contact yearbook advisor, Ms. Cokelet ercokeletmen@seattleschools.org.
