Name Change Form – due date was 11/10/25
Submit a yearbook name change by Monday, November 10. Have you recently transitioned? Do you use a different name at school than what Seattle Public Schools (SPS) has on record? Use this form if you are a current Hale student who would like the NHHS Heritage Yearbook to use a different name for you than the name that is registered with SPS. The yearbook publishes students’ first and last names as they appear in The Source unless a student submits this form. Questions? Contact yearbook advisor, Ms. Cokelet ercokeletmen@seattleschools.org.
