Athletic Paperwork

For athletic registration, Nathan Hale High School uses FinalForms, an online system, that boosts compliance and safety. FinalForms allows you to complete and sign forms from your computer, tablet, or mobile device.

You may log in any time to update information including emergency medical contacts and information. You will be required to sign your forms once per year and any time you make an update.

We ask ALL parents/guardians of athletes to use FinalForms:

Parents follow the prompts to create your account, manage your students and sign your forms. A parent must create their FinalForms account first, before your student is able to create their account.

Student physical exam form and receipt showing payment of ASB fee should also be uploaded onto FinalForms to complete your student’s file. Students needing to complete a physical can schedule to have that done in the Teen Health Center located in the building.

Spectator Expectations and Standards of Sportsmanship

Spectators are an invaluable component of our student-athletes’ experiences at athletic contests. As guests of the school district, and as a condition of such permission, spectators must comply with the school district’s rules and policies. These expectations are to protect the rights of students to participate without fear of interference, and to permit the administrators and officials of school athletic activities to perform their duties without interference.

The following are expectations for all spectators’ conduct to promote positive sportsmanship:

As member schools of the WIAA, the Metro League is committed to promoting fairness, equity, and safety during all athletic contests.

All parents, guardians, and supporters will conduct themselves in a positive sportsman-like manner with the shared understanding interscholastic competitions should be played a safe environment which promotes learning and growth for all student-athletes.

Negative remarks, vulgar language, and unwarranted physical interaction are direct violations of positive sportsmanship and could result in the following:

  1. Removal at a competition
  2. Inability to attend future competitions for all or a portion of the season
  3. Trespass admonishment and inability to attend all future competitions

The following are guidelines for all spectators to promote positive sportsmanship:

  • Spectators will respect all coaches, umpires/officials, administrators, and other spectators
    • Do not engage in physical or verbal intimidation, or abuse towards any player, official, umpire, coach, or spectator
  • Spectators must always remain off the playing surface, including halftime
    • Respect all facilities and equipment
  • Spectators should remain in their designated seating section
    • Comply with any directives from facility supervisors, administrators and/or event staff
  • Spectators will cheer for their team in a positive manner
    • Do not to use disparaging remarks towards the other team(s)
  • Spectators will:
    • Not use artificial noisemakers
    • Remain fully clothed
    • Not throw any objects into stands or onto event surface

FinalForms Reminders

  • Starting August 11, 2025, families must be enrolled with Seattle Public Schools to have an active FinalForms account
  • Athletic forms are valid for one school year only
  • We are using FinalForms to register online for high school athletic programming. If you previously had an account, you can log in and all your info will still be there!

    If you have not created an account, please follow the instructions below. A parent/guardian must create their FinalForms account first, before the student is able to create their account.

    To accurately complete your student’s account, it will be helpful to have the following information available to complete your student’s online registration:

    • Health Insurance Information (Company and Policy Number)
    • Basic Medical History and Health Information
    • Doctor and Dentist Contact Information
    • Physical Paperwork (WIAA Exam Form Required)

    Parents/Guardians

    1. Go to the SPS FinalForms website
    2. Select “Click Here for a Confirmation Email”
    3. Check your email for a confirmation link, and click confirm your account in the email text.
      • You will receive an email within 2 minutes prompting you to confirm and complete your registration. Remember to check your spam folder.
      • Reminder: FinalForms will use the same parent/guardian email on file in PowerSchool
    4. Create your new FinalForms password and click confirm account.

    Students

    1. Students will use their district (SPS) email account to log into FinalForms
      • Reminder: Students new to the district (or participating as Athletics Only) should contact their school for log-in support

    Physicals are valid for 24 consecutive months.

    • The History document (Page 1) should be completed and remain with your provider
    • The Exam document (Page 2) should be completed and remain with your provider
    • The Medical Eligibility document (Page 3) should be the only item submitted to your school

    Only the WIAA Physical Participation Form (linked above) is accepted.

    The FinalForms platform has a dedicated section to upload a completed preparticipation physical MEDICAL ELIGIBILITY form.

    Student-athletes must have health insurance to play a sport. If not covered by a parent/guardian’s insurance plan, insurance can be purchased through SPS.

    SPS Student Insurance Enrollment

    If families are unable to access this online platform, they should contact their high school athletic director and/or athletic support staff. Computers, as well as physical packets for uploads, can be made available for families to complete registration online.
